Independent VoIP provider Packet 8 was reported to have a power outage resulting to the failure of the company's entire VoIP network. The outage is said to be DNS-related.
According to the company, Registry.com messed up all of the DNS configurations pointing to Packet 8 when it tried to renew three of its domains. More information can be found here at DSLReports.com
